Step 1: Contact doctor for a phone screening
A phone screening will be the first contact where the doctor will obtain some basic information and a preliminary decision will be made in regards to further evaluation. If the doctor determines that the services required are not available or appropriate for their practice, they will do their best to provide referrals to an appropriate treatment location. Step 2: Submit registration packet
In order to set up a visit please first call or email the doctor about availability and goodness of fit together. After this has been done, please fill out, and return both the Conditions and Registration material by whatever method the doctor has avaialbe (email, fax, mail). (go to Doctors)
NOTE: Each physician may elect to use different methods of correspondence, so please ask the individual provider for the location and details of their paperwork.
Step 3: The doctor will call you to set up a time and place to meet
The next step is an an evaluation at the office which will allow doctor and patient to discuss the treatment needs in more detail. This can often be accomplished in a single visit but will, at times, take as many as three sessions. At the conclusion of this evaluation period, the doctor will, in collaboration with the patient, provide treatment recommendations. (go to Doctors)
